Ball & Chain Help

So, I have tried to figure it out, but I don't know what I'm missing. The test level that comes with the Ball & Chain GFX in Red Yoshi's Pack doesn't explain how to make it. I've tried and it fails miserably. So if anyone can give me a crystal clear tutorial on how to make it work then could you please send one to me in Notepad.

If you can, then I will SMBX rip a sprite sheet for you as a thank you, you choose whatever sheet you'd like of course.
